Anesthesia and Analgesia for the Critical Patient

چکیده
Pain is present with many diseases as well as in association with surgical and traumatic conditions. The demonstration of pain is not always obvious; therefore, an animal should be assumed to be experiencing pain in any condition expected to produce pain in humans. The assessment and control of pain is an art as well as a science. Clinicians should keep in mind that the art of pain management is a continual learning experience requiring assessment and therapeutic adjustment for individual animals even when they are undergoing similar surgical procedures. Therefore, standard or ruleof-thumb analgesic and anesthetic protocols are not always appropriate. If we accept that animals can experience pain, then how do we determine if a ferret or rabbit is or is not painful? It is likely that the tolerance of pain by ferrets and rabbits varies greatly between individuals, as it does in other species. This coupled with ferrets’ innate ability to mask significant disease, and probably pain, make it difficult to assess pain. Compared with dogs and cats, very few investigations have been carried out on the assessment and alleviation of pain in ferrets and rabbits. Most likely, as in cats, the mainstay of pain assessment in ferrets, rabbits, and birds appears to be behavioral. There are certain behaviors that are commonly seen in ferrets and rabbits suffering acute trauma or postoperative pain. Such ferrets and rabbits are often depressed, immobile, silent, and appear and distanced from their environment. They do not respond normally to petting or attention; ferrets and rabbits tend to hide when experiencing pain. They may also hyperventilate. In the my clinic, I have found that ferrets and rabbits have a very similar response to pain as is seen in cats.

Patient controlled analgesia versus conventional analgesia for postoperative pain
Purpose: Patients may control postoperative pain by self-administration of intravenous opioids using devices designed for this purpose (patient controlled analgesia or PCA). This study set out to determine whether any of the two opioid administrations (i.e. PCA or conventional analgesia) would provide superior pain relief among patients undergoing laparoscopic cholecystectomy or not. ...

متن کاملComparison of effects of 5% Lidocaine and 5% Meperidine plus 5% Lidocaine on complications and duration of postoperative analgesia for cesarean section
Introduction: Nowadays, spinal anesthesia is associated with few complications in many surgical practices especially the elective caesarean which is taken as a suitable replacement for general anesthesia. Different drugs are used for spinal anesthesia. This study aims to compare lidocaine 5% in combination with meperidine 5% plus lidocaine 5% for spinal anesthesia in non-emergency cesarean pati...
متن کاملEffects of administration of dexmedetomidine with intrathecal bupivacaine on analgesia after femoral and tibia orthopedic surgery: A double-blind randomized clinical trial study
Introduction: Various drugs are used as adjuvants for various purposes such as increasing analgesia, reducing analgesic complications and generally improving the quality of anesthesia with topical anesthetics. The use of alpha-2 agonists has recently been considered. One of these drugs is dexmedetomidine, which has been studied recently. The aim of this study was to evaluate the effect of dexme...
متن کاملComparing Different Epinephrine Concentrations for Spinal Anesthesia in Cesarean Section: A Double-Blind Randomized Clinical Trial
Background: Although various anesthetic techniques can be used in different kinds of surgeries, spinal anesthesia has received considerable attention for the lower abdomen and lower extremities surgeries and cesarean section. This study aimed at comparing the effect of adding epinephrine 1:1000 and 1:10000 to lidocaine and fentanyl in spinal anesthesia on the prolongation of paralysis, analgesi...
متن کاملIncidence of awareness with recall during general anesthesia
Background: Experience of awareness and recall during general anesthesia can be very distressing for patients. There is no doubt that surgery pain is the most distressing feature of awareness. This condition can cause neurosis in patients. If patient be aware during surgery and be able to hear some comment, she or he senses weakness, paralysis, feeling of helpless and anxiety, panic and death. ...
